Each client will have a dedicated Partner Success lead. You should be meeting and talking to these people often. Beyond these day-to-day contacts, we have a full team of clinical documentation educators, an outreach team to schedule Annual Wellness Visits and Foundational Care™ appointments, analytics and underwriting to bring you actionable info on your patients, and experienced RNs and Care Coordinators to support eligible high-needs patients between their appointments.

More frequent interactions with their PCP. We’ve found that providers in the most successful ACOs see their Medicare patients an average of five times per year. We work to drive engagement between appointments through our Foundational Care service which is offered by Wellvana’s RNs to eligible high-needs patients in full-risk ACOs.
